Medical Consent
Your child will be coming home with a Medical Consent form that needs to be filled out and turned in by Sept. 13th. If we do not have this on file, we will be unable to attend to any medical needs your child might have, such as a band-aid, tummy ache or a scraped knee. You will have to come to school yourself to take care of these needs unless this form is returned to us to have on file.
Putnam County School personnel understand that your child’s health and well-being are of the utmost importance to you, as well as the school system. Therefore, we would like to emphasize the significance of your presence in our office as soon as possible if your child requires advanced medical assistance from (diabetic care, care for seizures, acute allergies, tube feeding, cystic fibrosis, etc.) our school nurses and/or other school personnel.
